Case Management
Case Management serves people who are living with HIV/AIDS by providing comprehensive case management and support services.
Case managers help clients identify their needs through a comprehensive assessment and individualized service plans, and determine services needed based on financial and other eligibility guidelines. Case managers provide advocacy, HIV/AIDS education, and support to people living with HIV/AIDS.
The ultimate goal of case management is to help clients become self-reliant and self-sufficient in managing their lives.
Case managers are the link to SAAF’s support services, SAAF’s housing services, and other community resources.
