Your initial intake appointment will be with our Early Intervention Specialist, who will discuss your HIV specialty care options in the community, individual needs, eligibility process and completion of Ryan White/ADAP enrollment paperwork.

What Can I Expect to Happen at My Intake Appointment?
Our Early Intervention Specialist will review and complete an intake to identify your immediate needs.
An Early Intervention Specialist will ensure that you are linked to medical care, assist with obtaining health insurance coverage if needed, review Ryan White/ADAP eligibility requirements for services, discuss the case management process and answer any additional questions.
What Should I Bring to My First Intake Appointment?
Please bring the following documents for your intake appointment:
- Proof of income (Last four paystubs; SSI, SSDI, Pension award letter; unemployment information; etc.)
- Proof of Pima County residency (Current utility bill; Lease/Mortgage agreement; etc.)
- All medical insurance coverage information
- Government issued photo identification
- Medical provider information (if applicable)
- List of current medications
- Proof of HIV status (if available)
- Results of last lab work (if available/applicable)
- Last and next medical appointments (if applicable)
- Questions you would like answered
