Family Nurse Practitioner

Scope of Work

SAAF’s Family Nurse Practitioner will be part of SAAF’s strategic growth into clinical services, creating the region’s first health center focused on LGBTQ+ affirming care and all marginalized populations. This is an exempt position and reports to the Director of Clinical and Prevention Services.

The SAAF strategic plan that includes providing direct clinical services to our growing community. These services will be provided free of charge, allowing us to reinvent the wheel away from a fee-for-service model of care to an innovative service line that can integrate with the community, with SAAF’s current services to over 2000 clients, and truly meet community needs.

Services will target SAAF’s current clients, including those living with HIV and those utilizing SAAF’s prevention / testing / PrEP navigation services and youth services; and will focus on providing primary care to all marginalized members of the community, LGBTQ+ members of our community, and those who have historically been under-served by our health care infrastructure.

Functional Responsibilities

Provide full spectrum primary care services for patients 13 years to adults, including:

  • Chronic care visits
  • Prevention / well visits / pap smears
  • PrEP / PEP (training can be provided if experience limited)
  • Gender affirming care (training can be provided if experience limited)
  • Acute care for established patients
  • Medication Assisted Treatment for Substance Use (Suboxone)
  • Curative Hepatitis C treatment
  • Reports to the Director of Clinical and Prevention Services

Minimum Qualifications

  • Unencumbered AZ license or AZ-eligible for licensure in Advanced Practice Nursing; current prescribing privileges including a DEA license for controlled substances.
  • Current CPR / BLS
  • Three years substantive experience providing full spectrum primary care services to peds and adults, and ability to practice unsupervised/ independently.
  • Substantive experience and comfort providing care for and serving the LGBTQ+ community and marginalized populations.
  • Comfortable with providing care and serving communities of color, low-income communities including those experiencing homelessness, communities with IVDU, undocumented communities, etc., and in providing harm reduction services.
  • Willingness to work some evenings/nights and weekends each month based on SAAF programs and community outreach.

Preferred Qualifications

  • Able to conduct full patient history and physical in Spanish.
  • Flexibility and willingness to innovate — SAAF Health does not see patients every 15 minutes, we are striving to provide time needed for providers to care holistically for patients.
  • Willingness to work after hours if needed to help support those that utilize the syringe access program.
  • Knowledge of medication-assisted treatment for substance use.

Compensation

Exempt Salary: $104,261.00 – $132,933.00 DOE

Eleven (11) paid holidays, two (2) half day paid holidays, one (1) paid floating holiday, twenty (20) days of paid vacation per year to start, in addition to 40 hours of sick leave.  Health, dental, vision, and life insurance; 401(k) with matching, long-term and short-term disability insurance. Physical Requirements

This job is a Sedentary Work position, as defined by the Social Security Administration, CFR. CFR § 404.1567.

Physical exertion requirements: (a) Sedentary work. Sedentary work involves lifting no more than 10 pounds at a time and occasionally lifting or carrying articles like docket files, ledgers, and small tools. Although a sedentary job is defined as one which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required occasionally and other sedentary criteria are met.
